The PURE study asked a simple question across 17 countries: does how hard you can squeeze predict how long you live? It does, and better than one of the numbers your doctor measures every visit.

The PURE finding

Leong and colleagues measured grip strength in 139,691 adults aged 35 to 70 and followed them for four years. After adjustment, every 5 kg decrease in grip strength was associated with a 16% increase in all-cause mortality, a 17% increase in cardiovascular death, and higher rates of heart attack and stroke. Grip strength was a stronger predictor of mortality than systolic blood pressure.

Grip is not magic on its own. It is a proxy for total muscle mass and strength, which is a proxy for how well the body has been maintained. That is precisely why it is such a useful baseline test: it is one number that summarises a lot.

Why a dead hang, not a dynamometer

A dynamometer measures peak grip force; a dead hang measures grip endurance plus shoulder and scapular integrity, in a movement you can do on any bar with no equipment to buy. The two correlate well, and the hang has the advantage that it is also training. Most adults who cannot hang for thirty seconds have a strength gap they can feel.

Reference points

Recreational fitness standards put a 30-second hang as a reasonable floor for adults, 60 seconds as solid, and 90 seconds or more as strong. Women, lighter people and people with smaller hands are advantaged by bodyweight and disadvantaged by hand size in roughly equal measure, so the standards hold up better than they might appear. The number that matters most is your own, six weeks apart.

Unknown. Grip is a marker of overall strength; the sensible response is full-body strength training, of which hanging is one small part.
How long should I be able to hang?
Thirty seconds is a floor for most adults; sixty is solid; ninety plus is strong. Track your own number every six weeks.
What if hanging hurts my shoulders?
Stop and work on scapular control first; an active hang with shoulders pulled down is safer than a passive dead hang for many people.
